You may qualify to be a patient.

The application process is very simple.
Just follow the three steps below and we look forward to seeing you soon.

All applicants applying for the first time must apply in person.

Determine If You Qualify

To be eligible for services at Bethesda, applicants must fulfill the following requirements:

Age: Medical applicants must be between 18-64 years old, while dental applicants must be 18-65 or older.

Employment: Applicants should either have a paid job or engage in significant unpaid work, such as caregiving, volunteering, or attending school.

Uninsured: Those with any form of medical or dental insurance are not eligible for the respective services at Bethesda.

Apply In Person

To enroll or renew as a patient, visit an Admissions Advisor at the clinic. Unsure of eligibility? Consult with an advisor. Admissions operates on specific days and times weekly:

Monday: 9AM – 12PM
Tuesday: 10AM – 1:30PM
Wednesday: 1PM – 4PM
Thursday: 1PM – 4PM
Friday: 9AM – 12PM

Patients need to requalify every two years.

Bring Your Documentation

You will need to bring ALL of the following documents to Admissions:

  • A valid photo ID
  • Proof of income (recent paystubs, paid worker letter or awards letter)
  • Latest filed tax return (Form 1040 pages 1 and 2)
